- Completed application form with one/two passport size photograph
- Identity proof - Driving License/PAN Card/Passport/Voters ID card
- Address Proof (Ration card/latest telephone or electricity Bill/Passport/Trade license/Lease agreement/Sales Tax certificate)
- Age proof-Passport/PAN card/Voter’s ID
- Last 6 months’ bank statement
- Proof of continuation

Documents Required for self-employed (partnership farms & Private Ltd. Co.)
- Identity proof of the Private Limited Company: VAT/Sales tax / Service tax / IT return/ Excise registration of the concern, Electricity/Water / Shops and Establishment Registration, PAN ID /Municipal tax receipt in the name of the concern, Memorandum & Articles of Association
- Individual Identity Proof: Identity Proof to be submitted for the authorized signatories and two directors (that includes the managing director) (any one of the following- PAN card, Driving license, Voter’s identity card and Passport
Documents required For self-employed individuals (professionals)
- Proof of identity of the sole proprietorship
- IT return/PAN ID of the concern or Water, Electricity bill, Municipal tax in the name of the concern
- Individual identity proof for the sole proprietor: Driving license, Voter’s identity card, Passport, PAN card.
- Proof of residence for the sole Proprietor: Ration card, Passport, Driving license, Voter’s identity card
Documents Required self-employed individuals (non-professionals)
- Proof of identity of the sole proprietorship
- Individual identity proof
- Income Tax return for 3 years
- Sales tax returns for 3 years
- Copies of age-wise book debt, periodic stock and creditors’ statement ( last 3 months)
- Last 6 months’ bank statement of the primary banker/bank statement copy for the term loan
