Home Quarantine for Old People

Old patients that are aged around 60 years and people with co-morbid conditions such as heart condition, cerebrovascular disease, chronic lung or liver or renal disorder, hypertension, diabetes, etc. shall only consider home isolation post a correct assessment by the treating medic.

If the situation is that there is a fall in oxygen saturation levels or breath, the person should consider hospital admission and seek instant consultation with the treating physician or surveillance team.

Place and Method of Isolation

The patients must isolate themselves keeping themselves away from the other members of the family, stay in a particular room that is far away from other members, particularly, from elderly people and people with co-morbid conditions such as disorder, renal disease, hypertension, diabetes, etc.

The patients should remain in a well-ventilated room with cross-ventilation, the windows should be kept open allowing fresh air to enter the room and should use a triple-layer medical mask.

The mask should be discarded after 8 hours of usage or earlier if they turn wet or visibly soiled. If the caregiver enters the room, both the caregiver and the patient must ensure using the N95 mask. The masks should be discarded only after disinfecting them with 1% hypochlorite.

Medication

If the fever is not getting in control with a maximum dose of the tab, consider giving paracetamol 650 mg four times a day to the patient. Consult the treating medic, who may consider advising other medicines such as non-steroidal anti-inflammatory (NSAID) (Naproxen 250 mg 2 times a day), Ivermectin (200 mcg/kg once a day), to take on an empty stomach for 3 to 5 days consecutively.

The patients can also consider performing warm-water gargles or taking steam inhalation 2 times each day.

Inhalational Budesonide can be considered to be given to the patient through inhalers with spacer at a dose of 800 mcg two times a day for 5 to 7 days in case the symptoms such as fever or cough persist over 5 days. The choice to give Remdesivir or the other investigational therapy must be taken into consideration only by a medical professional and it should be provided only in a hospital.

 Besides, in case the symptoms such as constant fever or intensifying cough remain more than 7 days, consult the treating medic for treatment with low-dose oral steroids.

As per the guidelines, the patients must communicate with a treating physician and promptly report back to the latter just in case of any worsening. They must continue the medications for other comorbidities after consulting the treating doctor.

Guidelines for Caregivers

A caregiver should be present to supply care on a 24×7 basis to the patient. A communication link between the caregiver and the hospital is essential for the whole course of home isolation. If the patient is affected by an immune-compromised status such as transplant recipients, cancer therapy, HIV positive, etc. then s/he is not advised for home isolation and shall only be allowed home isolation after a correct assessment.

The caregiver and every close contact with mild or asymptomatic Covid-19 cases should consider ingesting Hydroxychloroquine prophylaxis in accordance with the protocol and as prescribed by the treating physician.

What to Ingest and Method of Ensuring Hygiene

The patient must take rest and drink a lot of fluids to conserve ample hydration, follow respiratory etiquettes all the time, not share personal things with others.

Besides, the caregiver should ensure cleaning of surfaces in the room that are touched often such as doorknobs, handles, tabletops, etc. with a 1% of hypochlorite solution. Additionally, self-monitoring of blood oxygen saturation with a pulse oximeter is also strongly suggested.

When to Finish Home Isolation

The patient under home isolation can be discharged post at least 10 days have passed since the symptoms came on board or from the date of sampling for an asymptomatic case and if the fever has not taken place for 3 consecutive days. There is no need for testing after the house isolation tenor is finished.

The revised guidelines by the Government of India stated that the mild or asymptomatic cases are laboratory-confirmed cases that are not undergoing any sort of symptoms and having oxygen saturation at room air of 94%, while the mild cases are the patients with upper tract symptoms or fever without shortness of breath and having oxygen saturation at room air of around 94%.
